帕金森病(Parkinson's disease,PD)是一种常见的、慢性进行性神经系统退行性疾病,主要影响运动功能,多在中老年人群中发病。PD其核心特征是大脑中特定区域的神经元逐渐退化,即中脑黑质多巴胺能神经元的显著丢失以及细胞内路易小体(Lewy Bodies)的形成[1]。
在 PD 患者中,SNpc 的多巴胺能神经元会进行性死亡(病程中可能丢失 50%-70% 以上),导致纹状体多巴胺水平显著下降,打破基底节环路的平衡(如抑制性递质 γ- 氨基丁酸(GABA)功能相对增强),最终引发运动症状(震颤、肌强直、运动迟缓等)[2]。
正常α- 突触核蛋白(α-synuclein, α-syn)是可溶性小分子蛋白,主要分布于突触前膜,参与突触囊泡释放和突触维持;而在 PD 中,α-syn 发生错误折叠(形成 β- 折叠结构),聚集成不溶性纤维,最终形成路易小体或者神经突。他们不仅会直接损伤神经元结构(如破坏突触功能),还会通过多种途径诱发细胞死亡(如抑制蛋白酶体活性、干扰线粒体功能、激活炎症反应等)[3]。
