胃腺癌(Gastric Adenocarcinoma)

胃腺癌是胃癌中最常见的病理类型(约占所有胃癌的 90%)。胃腺癌的发生是从正常胃黏膜开始,经历"慢性炎症→萎缩性胃炎→肠上皮化生→异型增生(上皮内瘤变)"的渐进式发展过程,每个阶段均伴随分子水平的异常累积。核心机理可分为外部诱因启动、慢性炎症驱动、分子遗传异常三大环节[1,2]

1. 外部诱因:启动胃黏膜损伤的 “始动因素”

外部环境因素是胃腺癌发病的核心诱因,其中幽门螺杆菌(Hp)感染是最明确的 Ⅰ 类致癌因素(WHO 认定),其他因素通过协同或叠加作用加剧损伤,如长期摄入高盐饮食和遗传易感性[3]

2. 慢性炎症驱动
(1)黏膜屏障破坏与上皮萎缩

炎症因子(如 IL-1β、TNF-α)抑制胃黏膜上皮细胞的增殖与分化,导致胃底腺萎缩(胃酸分泌减少),进一步降低黏膜对损伤因素的抵抗能力;

(2)肠上皮化生(IM)

为适应胃酸减少的环境,胃黏膜上皮被肠型上皮替代(如出现杯状细胞、吸收细胞),此过程伴随基因表达模式改变(如 CDX2、MUC2 等肠上皮标志物表达上调),肠化生上皮的增殖活性更高,更易积累突变;

(3)异型增生(上皮内瘤变)

肠化生进一步发展,上皮细胞出现形态和结构异常(如核大、核仁明显、排列紊乱),细胞增殖失控(如 Ki-67 指数升高),但尚未突破基底膜(属于 “癌前病变”),此阶段是逆转癌变的关键节点,若未干预,约 5%-10% 的中重度异型增生会在 5 年内进展为腺癌[4]

3. 分子遗传异常

随着黏膜损伤的累积,胃黏膜细胞逐渐出现基因突变、染色体异常、表观遗传改变,最终导致细胞获得 "无限增殖、抗凋亡、侵袭转移" 等恶性表型。

(1)基因突变与驱动通路异常

• RAS-MAPK 通路激活:约 10%-20% 的胃腺癌存在KRAS 或 NRAS 基因突变,导致通路持续激活,促进细胞增殖、抑制凋亡;

• PI3K-AKT-mTOR 通路激活:约 30%-40% 的胃腺癌存在PIK3CA 基因突变(编码 PI3K 催化亚基)或PTEN 基因失活(负调控 PI3K 通路),通路激活后可促进细胞代谢、血管生成及侵袭;

• TP53 基因突变:胃腺癌中最常见的突变之一(发生率约 50%-60%),TP53 是 "基因组卫士",突变后失去 DNA 损伤修复和诱导凋亡的功能,允许异常细胞存活并积累更多突变;

• EB 病毒(EBV)感染:约 10% 的胃腺癌与 EBV 感染相关(EBV 相关性胃癌),EBV 通过编码 latent membrane protein 1(LMP1)等蛋白,激活 NF-κB、PI3K 等通路,促进炎症和细胞增殖,同时 EBV 感染可导致 DNA 甲基化异常(如沉默抑癌基因)。

(2)表观遗传改变

• DNA 甲基化:如抑癌基因p16INK4a、MLH1的启动子区高甲基化,导致基因沉默(无法表达蛋白),分别失去细胞周期调控和 DNA 修复功能;

• 组蛋白修饰:如组蛋白去乙酰化(HDAC)导致染色质浓缩,抑癌基因转录受抑制。

(3)微环境重塑

• CAFs 分泌胶原、纤维连接蛋白等 ECM 成分,形成物理屏障,阻止免疫细胞浸润;同时分泌 IL-6、TGF-β 等细胞因子,促进肿瘤细胞增殖和侵袭;

• 免疫抑制细胞(如 Treg)分泌 IL-10、TGF-β,抑制效应 T 细胞功能,导致 "免疫逃逸",允许肿瘤细胞生长[5]
